Considering the above form is being submitted with due permission from the mentioned guardian, by proceeding with the form's submission, the student/guardian hereby declares that the above details are true to his/her knowledge and that the mentioned student will be attending the workshop on 14th January, 2016. Also, he/she understand the fact that the imfundo will not arrange for transportation to and from the workshop venue and that the student will adhere to the schedule disclosed above.
